Referral Checklist

Here is what you need to order and send on to us in order to refer a patient:

  1. Antenatal Record 1 and 2 (see Ontario College of Family Physicians site for current forms)
  2. First trimester ultrasound, 18-20 week (second trimester) morphology ultrasound if doing shared care
  3. Enhanced first trimester screen (eFTS) or non-invasive prenatal testing (NIPT) if available
  4. The following lab work:
    • complete blood count (CBC)
    • tests for ABO and Rh incompatibility (ABO/Rh)
    • antibody screen
    • urine culture and sensitivity test (urine c/s test)
    • HIV test
    • hepatitis B test
    • Venereal Disease Research Laboratory (VDRL) test
    • rubella immunity test
    • gonococcal conjunctivitis (GC) and chlamydia swab results
    • last pap smear results
    • any other patient-relevant lab work
